Referral for Services
Individuals or a referral source can access services by calling any one of our offices. A referral can either be made by contacting the preferred office or by submitting the online referral form. Once a referral is received, a staff member will complete a brief intake over the telephone and an appointment will be scheduled for a diagnostic assessment. We accept various insurances, please refer to our Accepted Insurances page for a complete list. Please contact us if you have any questions.
In the event of an emergency or need to speak with a clinician immediately, please visit your nearest emergency department or Netcare Access (http://www.netcareaccess.org/). If you are experiencing a crisis or have an urgent need, please contact National Suicide Prevention Lifeline at 1-800-273-8255 (TALK). Additionally, there is a Crisis Text Line that serves anyone in any type of crisis. It is available by texting “HOME” TO 741-741.

To make a referral for an Evaluation (such as Psychological Evaluation, Focal Evaluation (AOD Assessment, Anger Management Assessment, ADHD Assessment, or other formal assessment) please contact our Executive Assistant, Mary Miller at 740-687-0042.
